Under the EYPP, coming into force in April, early years settings will be able to claim an additional 53p an hour for every disadvantaged three- and four-year-old in their care eligible for the funding.
The publication of the model form and letter follows concerns from early years providers about how they will communicate the importance of the extra money to parents and encourage them to find out if their children are eligible.
To help early years providers identify children eligible for the EYPP, the Department for Education has created a model registration form, which can be printed off.
The form asks for parents’ contact details, National Insurance number, information about their family income and details of any benefits they receive.
The DfE recommends that nurseries ask all parents and guardians, regardless of family income or circumstances, to compete the form when they enrol their child.
To explain to parents what the Early Years Pupil Premium (EYPP) is, there is also a template letter providers can use.
The letter explains what the EYPP is, children that are eligible, and ways in which nurseries can use the funding to improve the quality of their setting, for example paying for additional training for staff.
Providers are free to amend the documents as they wish.
Once completed, the DfE says that early years providers should share the completed forms with their local authority so the necessary checks can be made and settings receive the funding they are entitled to under the scheme.
